A calculator on top of formsOur doctors and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure that it is performed in the most responsible manner possible. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Please understand that treatment suggestions are always based on patient needs and NEVER determined by insurance benefit reimbursement.

Initial Exam / Emergency Exam / Single-Visit Payments

For your initial exam, emergency exam or other single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders and credit card payment (MasterCard, Visa, and Discover).

If you have any concerns or questions, please ask our staff. We can be reached at Champaign Location Phone Number 217-398-4867 in Champaign or at Charleston Location Phone Number 217-345-4867 in Charleston. Our offices are located at 3002 Crossing Ct. Champaign, IL 61822 and 1720 South 18th Street, Charleston, IL 61920.

Active Treatment Plans / Multiple Visit Payment Options

Once your treatment plan is determined, the fee will be discussed. We offer several different payment options to assist you in financing your dental health. Arrangements will be made for payment with our front desk staff.

Option 1: Full Payment of Complete Treatment Plan in Advance
We offer a 5% courtesy fee reduction for payment in full (cash, check, money order, MasterCard, Visa, or Discover only) before treatment is rendered. Our 5% courtesy fee reduction does not apply to Dental Implant Treatment or CareCredit. Selection of this option gives you the maximum savings on the cost of your treatment plan, because the billing expenses we save are passed on to you in the form of a 5% discount.

Option 2: Pay as You Go
Payment for your procedure at each individual treatment appointment for charges incurred that day is due at time of service.

Option 3: CareCredit
For patients who would prefer an extended payment plan, outside financing is available to those who qualify. CareCredit lets you begin your treatment immediately, then pay for it over time with low monthly payments that are easy to fit into your monthly budget. With CareCredit you pay no up-front costs, no pre-payment penalties and no fees. Plus, CareCredit offers a revolving credit line for additional treatment or add-on charges, without the need to re-apply.

This plan allows 6 months of no interest payments for a minimum treatment plan of $300, or 12 months of no interest payments for a minimum treatment plan of $2000. Extended payment plans beyond 6 months are also available at reasonable interest rates. Please call 1-800-365-8295 or visit online at www.carecredit.com for additional information.

Patients applying to CareCredit must allow ample time for pre-approval before scheduled appointment(s). At the time treatment is rendered, you will be asked to present two forms of identification per CareCredit policy (Drivers License, Government issued Identification Card or Major Credit Card).

We are happy to discuss all payment options in further detail at your scheduled appointment.

Dental Insurance

Professional care is provided to you, our patient, and not to an insurance company. Your dental insurance plan is a contract between you and your insurance company. We are happy to prepare and file insurance claims as a courtesy that we extend to our patients to facilitate prompt reimbursement, and we will gladly use our resources to assist you in seeking reimbursement to the full extent permitted under your policy.

Our staff deals with many different insurance companies, and some organizations offer multiple dental and medical plans. Periodically, your place of employment might change benefits. As a courtesy, we ask that you keep us informed of any change to your insurance coverage. It is important that all of our information about you and your insurance is current.

If you have dental insurance, we can submit your treatment plan for a pre-estimate of benefits. Regardless of insurance, the payment responsibility rests with you, the patient, not your insurance company, as an insurance preauthorization is not a guarantee of payment. Our policy is that payment is due in full at the time of service via cash, check, money order, CareCredit, or credit card payment (MasterCard, Visa or Discover). We will promptly submit the treatment procedures we perform to your dental insurance company and always direct that payment be sent directly to you from the insurance company.
